Place of Death: Residence. Date of Death: 12 September, 1919, at 11:42 p.m., approximately. Date of Attending Physician's Last Visit: 12 September, 1919.

IMMEDIATE CAUSE OF DEATH: Heart failure (myocardial infarction presumed). CONTRIBUTING CAUSE: Atherosclerosis; chronic indigestion of recent acute presentation; exertion. DURATION OF ILLNESS LEADING TO DEATH: Six days.
