Works accident book entry, Bracken Foundry
Entry copied from the rail-shop accident book for press No. 4, ruled in red pencil at the margin.
Date: Tuesday morning shift. East yard rail shop.
Job: axle yoke 6-B, north siding repair set.
Account entered by the office:
During wheel-press work the near flange gave way without warning and threw the yoke out of line before inspection had been completed. E.T. had not finished the gauge pass. J.V. ordered the press stopped and the piece moved to the back trestles for shop disposal.
Injury return:
One fitter removed from the press floor with hand and shoulder bruising. Bandage and rest ordered. No hospital cart required. Name copied to weekly works return.
Medical counter-entry:
Seen in surgery at 10:05. Fit for light duty after two days if swelling falls. No further notation required for the accident return.
Countersigned: Dr. Hale Smyth, works physician.
Office marginal instruction:
Do not append the cooling sheet or the held-part ticket to this page. The accident book is to carry the press-floor occurrence only. If the Board asks for timings, send the clean office cooldown figure and not the rough shop record.
